Preparing for Your Visit
To help ensure an efficient appointment, we recommend arriving a few minutes early and bringing the following items with you:
- Photo identification
- Insurance card(s)
- Referral information, if required
- Current medication list
- Relevant medical records or imaging studies
- Completed new patient paperwork, if available
Our team works closely with each patient to help coordinate care and ensure all necessary information is reviewed prior to treatment.
